I acknowledge that I meet the qualifications to participate in IPPL's Homebound program and that a doctor's certificate may be required. I give permission for IPPL staff to use my library card to check out materials on my behalf. I understand that I am expected to have materials ready for pick up when they are due, and that I am responsible for the replacement cost of lost or damaged items. Indian Prairie Public Library reserves the right to discontinue services at any time for any reason, including excessive material loss, scheduling conflicts, or other issues.
